Registering with the surgery

To register with the surgery:

We do understand that not all questions on our registration form are applicable to all patients. However, it is important we use the registration process to capture as much information as possible. This is to ensure that we are offering you the best standard of care and can signpost patients who may need extra support at the point of registration.

Please complete the registration form to the best of your knowledge with as much information as possible.

When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.

Children Registering Alone

Patient safety and safeguarding is paramount at Bedford Street Surgery, particularly with children. After continuing discussions, we find it crucial, for ease of communication and ensuring excellent patient care is provided for children, that at least one parent/guardian is registered at the practice alongside their child under the age of 13.

We understand this may be inconvenient for some however this decision has been made based on child safety. Should you wish to discuss any unforeseen circumstances why a parent/guardian cannot be registered alongside their child, please feel free to contact us online Ask the practice a question
