To register a new patient you will need to live within our practice boundary.

To register with the practice:

• use the NHS: Register with Furzton Medical Centre service, or
• use the NHS: Register with Bedford Street Surgery service

We do understand that not all questions on our registration form are applicable to all patients. However, it is important we use the registration process to capture as much information as possible. This is to ensure that we are offering you the best standard of care and can signpost patients who may need extra support at the point of registration.
