Please don’t bring in samples unless we have requested them.
If your doctor or member of our clinical team have asked you to provide a sample, please:
1. Please use the sample container provided by the surgery
2. Ensure your full name, date of birth and the date you obtained the sample are clearly marked on the container
3. Secure the lid tightly to prevent leaks
4. If you are unsure about anything, our reception team are always happy to help.
If the clinician has requested you return your sample to the practice to be checked on site (dip test) please ensure you inform reception of the clinician who requested this and return it within the time frame they have asked to ensure this can be assessed by them on the day.
If samples are provided outside of the times provided you may be asked to repeat the test as this has missed the times in which collections by the hospital are made
