New Patient Registration
When you register it will be with the practice not an individual doctor, as the practice is collectively responsible for your care. However you can ask to see a particular doctor but this will depend on their availability.
All patients registering will be asked to fill in a simple registration form as well as a simple medical questionnaire so that the doctor has some information about you immediately. If you have the following documents we would ask that you provide them but if for any reason you do not have these you are still able to register with our practice.
- Photographic identification - This can be a passport, photo driving license, ID card, citizenship card.
- Proof of address - This can be a recent utility bill, tenancy/rental agreement, mortgage/home ownership details or bank statement, these should be dated within the last two months of the date you come into the surgery.
You will also be offered a new patient health check with our health care assistant.
