All of our patients have an accountable named GP who is responsible for overall care. If you are unsure who this is please ask next time you visit the practice. If you have a preference to which GP that is please contact the practice.

 

Furzton Medical centre will introduce a new role of a nurse practitioner from April 2016 , if you have a minor illness we would recommend first you speak to your local pharmacy and if required book an appointment with our nurse practitioner there are a number of conditions she is able to prescribe for please check with reception upon booking.

Carers

Do you look after a family member, friend or neighbour who through illness, disability or frailty cannot manage without you? If so, you are a carer and we would like to help.

Carers need:

  • Recognition of their role
  • Opportunities for a break from caring
  • Care for their own health
  • Emotional support
  • Accessible information from reliable services

If you care, we care

At Bedford Street and Furzton Medical Centre we are aiming to identify all our patients who are carers. We are keen to ensure that all carers are aware of the help that is available, both locally and through specialist organisations and support services.

Advantages of being registered as a carer:

We can see you are a carer and the information will help your GP when planning your medical care and support

  • You will be invited for annual flu vaccination
  • We can provide contacts for carer groups and information

Our practice works alongside carers MK  who specialise in supplying support and advice for carers of all ages. If you have any questions please contact Tracy Blackmore (Carers Champion and Reception Manager at Furzton Surgery 01908 867064 .

Register as a Carer

Please complete the Carers referral form and bring to our practice , alternatively you can email the form to bedfordstreetsurgery@nhs.net

You will then be registered in our Practice’s records as a carer. With your permission we can also refer you to your local Carers’ Support organisation – Carers MK.

You will also need to give a Carers Medical Records Access Form  to the person you care for. By completing this form, the patient gives consent for their carer to access their medical records and information relating to their care e.g. test results. Please ask the person you care for if they wish to complete this form and return the completed form to the Practice’s receptionist.
