All of our patients have an accountable named GP who is responsible for overall care. If you are unsure who this is please ask next time you visit the practice. If you have a preference to which GP that is please contact the practice.

 

Furzton Medical centre will introduce a new role of a nurse practitioner from April 2016 , if you have a minor illness we would recommend first you speak to your local pharmacy and if required book an appointment with our nurse practitioner there are a number of conditions she is able to prescribe for please check with reception upon booking.

Do we have your upto date contact details ? please ensure you provide the practice with any changes

Test results

If you have a test in our practice the G.P will write to you if they wish to see you following your result or advise you to make a follow up appointment .

If you have consented to receiving SMS from the surgery you may also receive your test result indicator by text - Please ensure we have your up to date mobile telephone numbers

We  can not give you clinical information regarding your result over the telephone .Test results can be viewed online if your register for this service .

If the doctor wishes to see you urgently the Care Navigator team will telephone you to make an appointment

We would also write to you if your doctor wishes to see you for a routine appointment , please note routine appointments are classed as non urgent and can be booked in advance or on the day dependant on appointment availability . The GP's do not give test results through our message board system unless you are a housebound patient

If you require clinical information you need to complete a SAR ( subject access request ) which will then need to be processed . Please see section 10 for further information
